Outpatient Nursing Department

About the department

Our department includes general nurses working in outpatient clinics, specialist diabetes nurses (including a diabetic foot nurse), a home care nurse, healthcare assistants, a nurse specialising in nutrition therapy, and an optometrist.

The Diabetic Foot Clinic provides specialist care for patients in the following situations:

  • a patient with diabetes who has ulcers on the leg/feet
  • a patient with diabetes who has a history of ulcers on the leg/feet
  • a patient with diabetes who has foot deformities that increase the risk of ulceration
  • a patient with leg ulcers caused by other diseases (venous ulcers, etc.)


Please note: pedicures are not offered at the foot clinic. The service in this case is paid.

Registration and appointments

A valid referral from a GP or specialist, including relevant medical history, is required. Appointments with the nurse are free of charge for eligible patients.

The Diabetic Foot Clinic is held at the Merimetsa Health Center (Paldiski mnt 68a, 3rd floor)

The diabetes nurse provides specialist care and support for patients living with diabetes, including:

  • review of treatment plans and support with diabetes self-management
  • ongoing monitoring of health status
  • education and counselling to support effective diabetes management

Diabetes nurse appointments are held at Merimetsa Health Centre (Paldiski mnt 68a, 3rd floor) and Mustamäe Health Centre (Ehitajate tee 27, 2nd floor).

A doctor’s referral is required for the appointment to be free of charge. Without a referral, the consultation will be treated as a private (self-funded) visit.

Home nursing provides qualified nursing care in a patient’s home, on a doctor’s referral, for individuals recovering from acute illness, living with chronic conditions, or experiencing reduced functional ability, to support safe and effective coping at home.

Home nursing services can be provided to:

  • patients discharged from hospital who no longer require active medical treatment but need nursing care and are unable to attend their GP surgery due to physical or mental limitations
  • patients for whom home nursing may help prevent hospital admission or the need for long-term care
  • residents of care homes
  • individuals discharged from a nursing or rehabilitation hospital
  • patients receiving end-of-life care
  • patients with chronic conditions and limited mobility
  • individuals with reduced functional ability who require support to manage safely at home
  • patients whose nursing needs exceed the level of care that can be provided by a practice nurse

We provide the following nursing procedures:

  • blood pressure measurement
  • heart rate reading
  • body temperature measurement
  • administration of prescribed medication
  • intravenous therapy (including IV infusions)
  • wound treatment with bandaging
  • removal of sutures from wound
  • ostomy care (exchange, teaching)
  • administration of enemas
  • assessment of skin condition and prevention and management of pressure ulcers
  • replacement therapy
  • collection of samples and testing, including blood glucose monitoring using a glucometer
  • bladder catheterisation and permanent catheter care
  • nursing advice and guidance for patients and their families, including education on care, treatment, symptom management and day-to-day coping

Nursing care is provided in accordance with the volume of the contract concluded by West Tallinn Central Hospital and Health Insurance Fund (Tervisekassa) on a first-come, first-served basis within the City of Tallinn. Nursing care is delivered in close cooperation with the patient’s GP and, where appropriate, with local social services or the patient’s designated carer. Access to home nursing requires a referral from a GP or specialist. The referral should outline the reason for home nursing and specify the treatment procedures to be carried out at home. Please send your referral by email to koduoendus@keskhaigla.ee. For patients insured by the Health Insurance Fund, home nursing services are provided free of charge.

Nurse Elle Kalamägi provides advice and support on weight management and nutrition, including for patients who are underweight or overweight, those with medical conditions such as diabetes, and bariatric patients before and after surgery.
