Home nursing

About home nursing

Home nursing provides qualified nursing care in a patient’s own home. It is intended for individuals recovering from acute illness, living with a chronic illness, or experiencing limited functional capacity. Care is delivered on a doctor’s referral and aims to support effective coping at home.

Home nursing services can be provided to:

  • patients discharged from hospital who no longer require active medical treatment but need nursing care and are unable to visit their GP due to physical or mental limitations
  • patients for whom home nursing may help prevent admission to hospital (including nursing care, nursing home)
  • residents of care homes
  • patients discharged from a nursing hospital
  • patients receiving end-of-life care
  • patients with chronic conditions and limited mobility
  • patients with reduced functional ability who require support to manage safely and effectively at home
  • patients with a greater need for nursing care than can be provided by a practice nurse

Our nursing services include:

  • measurement of blood pressure
  • heart rate reading
  • body temperature measurement
  • distribution and administration of drugs
  • intravenous (IV) infusions
  • wound treatment with bandaging
  • removal of sutures from wound
  • ostomy care (exchange, teaching)
  • enemas
  • assessment of skin conditions and prevention, care, and treatment of bedsores
  • substitution therapy
  • blood sampling and blood glucose measurement
  • bladder catheterisation and permanent catheter care
  • nursing advice and guidance for patients and their families

Nursing care is provided within the City of Tallinn in accordance with the service agreement between the West Tallinn Central Hospital and the Health Insurance Fund. Services are allocated on a first-come, first-served basis. Care is delivered in close cooperation with the patient's GP and, if necessary, with the social worker or caregiver of the district government.

A referral from a GP or specialist doctor is required to access home nursing services. The referral should briefly outline the reason for home nursing and specify the treatment procedures to be carried out at home.

For patients insured by the Health Insurance Fund, home nursing services are provided free of charge.
